              The link to an advisor declaration form for my graduate program.

              After 6 years in the program, apparently somebody in the front office lost the folder containing everyone's advisor / committee declarations. One oops = lots of annoyed graduate students who have to get a bunch of signatures from notoriously hard to find faculty.
