No worse than the skills assessment form I use at work.
I had 2 people fill out the form, they marked that they were experienced at every commercial maintenance task under the sun and listed "I helped or watched someone else do it" for the details.
So I added the following line to the bottom of the form "Any falsification of information on this form is cause for immediate termination." and I make then sign and date the form.
